How to Log AFP

To Log an AFP you have to fill the form with three sections. These sections are:
 

  •  To: Food Alert

    In this section you need to fill the information relevant to your site
    Click "Next" for the next section.
     

  • Customer Detail

    In this section fill in the customer details who had food poisoning.
     
    You can also attach file relevant to AFP case.
     

  • Details of Notification

    In this section you need to fill the information regarding the food consumed that caused food poisoning.
     
    If the menu item is not in the list then click
     
    If you want Food Alert to investigate this AFP then select this option.
     
    Of this option is selected then someone from AFP department will contact you regarding the case.
     
    Click to save the case.
